LONG TERM CONDITIONS SUPPORT
We offer dedicated nursing support for individuals with long-term health conditions such as diabetes, heart health, and asthma care.

Our goal is to assist in achieving improved health through lifestyle changes or managing these conditions. Our nurses also provide guidance on wider health concerns, and appointments are often available on the same day.
Our team focuses on breaking barriers for individuals and whānau grappling with chronic diseases, ensuring they can easily attend medical appointments through advocacy and tailored care plans. We organise referrals to hapori support services as needed, guaranteeing holistic assistance. Through educational programs, we empower individuals to enhance their health literacy and take control of their well-being.
Our services, available onsite at He Waka Tapu or through home visits, are provided free of charge.
